What to bring to the DMV office
When you apply, you must prove your age, your identity and where you live. The proofs you bring, together with the fee you pay, decide whether your permit is Standard, Enhanced or REAL ID.
The full list of accepted proofs is in form ID-44 (How to Apply for a New York Learner Permit, Driver License, Non-Driver ID Card). DMV's online pre-screening tool tells you which forms and documents you need and lets you fill in a permit application to print and bring with you.
Bring the completed permit application (form MV-44), your proofs of identity, age and residence, and payment for the permit fee.
All proofs must be in English or come with a certified English translation.
If you need form MV-45, a parent or guardian's statement of your identity, your parent or guardian must come to the office with you. Do not sign MV-45 in advance: it must be signed in front of a DMV representative.
